A cognitive-psychological assessment is a valuable tool that provides useful information to both the individual receiving the assessment and their treatment providers. Dr. Moquin has extensive experience in assessing various cognitive and psychological symptoms. Among testing options she provides, Dr. Moquin's assessments can be instrumental in:
Symptoms such as problems in memory, attention and concentration, or cognitive flexibility do not automatically indicate any particular diagnosis. Conducting an assessment allows Dr. Moquin to rule out issues and ultimately identify the cause of the symptom(s).

2. You will need to complete registration paperwork and submit insurance card information, if applicable..
3. In addition to the registration paperwork, Dr. Moquin may ask for you to obtain collateral information, if applicable, such as family input, medical or school records to assist in getting to know you and your situation better.
4. Once submitted, we will schedule a one-hour initial diagnostic interview to gather historical data prior to testing. This can either be done virtually or in-person.
5. Following the initial diagnostic interview, the testing session will be scheduled. Testing standardly requires 3 - 5 hours, which can be done in 1 or 2 sessions; however, we recognize that not everyone is able to tolerate a lengthy testing session, and we will adjust accordingly.
6. Once testing is completed, a 30 - 60-minute follow-up will be scheduled to discuss the results of testing and to provide recommendations for additional care, based on the results of testing,
8. Two copies of the testing results will be provided to you at follow-up. We do not send copies to your providers.
Depending on your policy coverage, insurance may reimburse for testing; however, if your insurance does not cover testing or you prefer to not go through your insurance, paying out of pocket is an option.
Dr. Moquin reserves the right to require a $1500 deposit to cover testing in the event there is any concerns the patient's insurance will not cover the service. Once testing is completed and insurance reimbursement is received, the deposit will be returned to you.
If you choose to self-pay for services, the pre-testing initial diagnostic interview is a non-refundable $200 fee, due at the time of service.
Following the initial diagnostic interview, the out-of-pocket cost will be between $1200 - $2000, which includes testing, the follow-up session, and 2 copies of the evaluation report. The cost is contingent upon what is being assessed and measures needed to achieve diagnostic clarification. All fees will be discussed prior to the initiation of testing.
